Indications/Uses

Unresponsive left ventricular failure secondary to acute MI or of various aetiology. Severe or unstable angina pectoris.

Dosage/Direction for Use

2-12 mg IV every hr, may be increased up to 20 mg every hr.

Overdosage

Contraindications

Hypersensitivity to nitrates or nitrites. Low filling pressure, hypertrophic obstructive cardiomyopathy, constrictive pericarditis, cardiac tamponade, cardiogenic shock, circulatory collapse, aortic &/or mitral valve stenosis, severe hypotension (systolic BP <90 mmHg), head trauma & cerebral haemorrhage, increased ICP-associated diseases, marked anaemia, hypovolaemia, closed-angle glaucoma. Concomitant use w/ phosphodiesterase-5 inhibitors eg, sildenafil, tadalafil, vardenafil; soluble guanylate cyclase stimulator eg, riociguat.

Special Precautions

Patients w/ hypothyroidism, hypothermia, malnutrition, orthostatic syndrome; hypoxaemia & ventilation/perfusion imbalance due to lung disease or ischaemic heart failure. Development of tolerance. Monitor BP & pulse rate. Not to be used w/ phosphodiesterase-5 inhibitor-containing product eg, sildenafil, tadalafil, vardenafil. Avoid use w/ alcohol. May affect ability to drive & use machines. Severe renal & hepatic impairment. Not to be used during pregnancy & lactation. Childn.

Headache. Dizziness, somnolence; tachycardia; orthostatic hypotension; asthenia.

Drug Interactions

Potentiated hypotensive effects w/ phosphodiesterase-5 inhibitors eg, sildenafil; BP-lowering drugs eg, β-blockers, Ca antagonists, vasodilators, ACE-inhibitors, MAOIs &/or alcohol; neuroleptics & TCAs. Increased blood level & antihypertensive effect of dihydroergotamine. Hypotension w/ riociguat. Vasodilation w/ sapropterin‑containing medicine.
